MUST COMPLETE - School Health Services Form in Skyward

Bush Parents,
We need all of our parents to complete this form in Skyward AS SOON AS POSSIBLE.  It will only take you about 4 minutes to complete.  Based on SB12 for School Health Services, parents MUST provide schools with their decision regarding ROUTINE SCHOOL-BASED HEALTH SERVICES.  We have about 1/2 of our forms completed.

Here are the choices for the parents:
 
          -   Consent: Yes, I give consent for my child to receive routine school-based health services during the 2025–2026 school year. I understand that I may revoke this consent at any time in writing.
 
         -   Decline Consent: No, I do not give consent for my child to receive routine school-based health services during the 2025–2026 school year. I understand that I will be contacted in the event of an emergency.
